Project Foresight: identifying Veterans at risk of disease progression

Through the VHA Innovation Ecosystem’s Project Foresight, Cogitativo applied disease-progression analysis to identify clinically vulnerable Veterans, reporting scores across 9 million VHA beneficiaries as of May 2022 (Project Foresight overview). The work identified high-risk Veterans who had received no relevant care for their chronic condition in the preceding 12 months (Project Foresight overview).

The decision this supports is where clinical teams should look more closely and assess whether a timely response is needed. Risk identification informs that review; clinicians determine the appropriate care.

How can a program find Veterans at risk after deferred care?

The project overview describes analysis of clinical vulnerability across Type 2 diabetes, COPD, chronic kidney disease, and atherosclerotic cardiovascular disease.

Cogitativo presents this work as part of Visión’s clinical-foresight experience.

Potential care gaps for clinical review

A high-risk signal alongside an absence of relevant care identifies a potential care gap for clinical review. The clinical team determines whether care is needed and what response fits.

What this evidence supports

This company-reported experience documents risk identification at national scale (Foresight overview). The overview does not establish the clinical or financial effects of subsequent interventions (Foresight overview).
